The conversation around AI governance tends to live in policy documents and risk registers — abstract frameworks that sound comprehensive but offer very little operational guidance on what any of those things actually mean when you are designing an agent that will interact with customers, systems, and data in real time.
The gap between policy and practice
Governance frameworks tell you to ensure “transparency” and “accountability.” They rarely tell you what that looks like when an agent is autonomously triaging support tickets, reclassifying claims, or generating client-facing reports.
The result is a growing number of agents deployed without meaningful controls — not because teams are reckless, but because the guidance doesn’t meet them where they are.
The 5 controls that actually matter
After deploying agents across healthcare, financial services, and consumer goods, we’ve identified five controls that make the difference between an agent that scales and one that becomes a liability.
1. Scope boundaries
Every agent needs a clearly defined scope of action. What can it do? What can it not do? What triggers a handoff to a human? These boundaries should be encoded, not just documented.
2. Decision auditability
If an agent makes a decision, you need to be able to trace why. This isn’t just logging — it’s structured output that connects the agent’s action to the data it observed and the reasoning it applied.
3. Escalation paths
Agents will encounter situations they weren’t designed for. The question is whether they fail silently or escalate clearly. Design escalation as a first-class feature, not an afterthought.
4. Performance baselines
You can’t govern what you can’t measure. Every agent needs baseline metrics — accuracy, latency, exception rates — and automated alerts when those metrics drift.
5. Human-in-the-loop checkpoints
Not every decision needs a human. But certain categories of decision — high-value, high-risk, or novel — should require human approval. The art is drawing that line in the right place.
Making governance operational
The common thread across all five controls is that they’re implemented in the system, not just written in a policy document. Governance that lives only in documentation is governance that gets ignored.
